1. Complete your affidavit.
2. Sign, date & notarize your affidavit.
3. Locate your personal ID.
4. Scan your affidavit, bill & ID. Be sure to scan your Proof of Purchase if required. Save them as GIF, JPG or PDF file(s). (You can upload them as 1 file or as 4 separate files). Maximum size limit per file is 2MB.
5. Click on one of the browse buttons on the left to locate the file(s) on your computer.
6. Once you have selected the documents, click the "Submit" button to complete your upload.
